- Eligibility: Age 65 and older, current Medical Assistance recipient and qualifies for supportive services. To determine if you qualify for supportive services, you must request a Long-Term Care Consultation by contacting Public Health intake
- Supportive Services include: Home health aide, adult day services, respite care, personal care, home delivered meals, skilled nursing, and more
Home Care Services
- Eligibility: Persons with disabilities and special health care needs who qualify for Medical Assistance. Some recipients of MinnesotaCare may also qualify
- Services include: Professional nursing services, occupational therapy, physical therapy and other services
Personal Care Assistance (PCA)
- Eligibility: Current Medical Assistance recipient and qualifies for this service as determined by an in-home assessment completed by a public health nurse. If you have a health plan with your Medical Assistance, contact your health plan, and they arrange for the in-home assessment
